Reporting Options
If the concern you are reporting is an emergency, imminent danger and/or risk to health and safety, Dial 911. When it’s safe to do so, please complete one of the reporting forms below.
The Reporting Options below are available to share important information directly with the appropriate STCC offices. This is an internal reporting tool designed to address concerns and referrals in a supportive manner. The goal is to connect students with the necessary support and resources they need.
Forms
Members of the college community are encouraged to report the following:
- General Incident: Reporting potential violations of the Student Code of Conduct.
- Academic Dishonesty: Reporting alleged incidents of academic dishonesty. STCC’s Academic Honesty Policy and Student Code of Conduct contain detailed information related to academic dishonesty
- PAA/Title IX: The general STCC community and non-confidential employees should use this form to report concerns they have for a student who may be experiencing discrimination that violates the College’s Policy on Affirmative Action, Equal Opportunity & Diversity (PAA) or sexual harassment under Title IX.
- Share-A-Concern: Report any concern you may have about the physical, emotional, or social well-being of any STCC student. The report will be sent to the CARE Team for review and follow-up. You can also refer students who may benefit from referrals to on- and off-campus support and resources.

Individuals completing reports may not be notified about the specific action or interventions implemented. Select “Email me a copy of this report” prior to hitting “Submit” for confirmation that the report has been submitted. In some cases, you may be contacted for clarification and/or further information.
These reporting forms are not monitored 24/7 and do not notify emergency responders. The following are 24/7 crisis resources:
