Don't Have Documentation?

You can use the General Disability Verification Form

  1. Download the General Verification Form
  2. Ask a qualified health care provider to complete it
  3. Submit the completed form to ODS through AIM, email, or fax

What Happens After You Register?

After you submit your application and upload documentation, ODS will:

  • Review your application and documentation
  • Assign you an ODS counselor
  • Contact you through your STCC email to schedule an intake appointment

Register Early

Some accommodations take time to arrange. Start your registration as early as possible, preferably before the semester begins. Meeting with ODS early helps make sure your accommodations are in place when you need them.

Guidelines for Documenting a Disability

Any student with a documented disability is eligible to register for services. Your documentation should:

  • Confirm your disability
  • Come from a qualified professional
  • Be provided by someone impartial who is not a family member
  • Include current information about your disability when appropriate

You should submit documentation through AIM before your intake appointment.

Learning Disabilities

If you are requesting accommodations for a learning disability, you may submit:

  • An Individualized Education Program (IEP)
  • A 504 Plan
  • Intelligence and achievement testing
  • Other current documentation from a qualified professional

If you have an IEP or 504 Plan and testing, we recommend submitting all of your documentation. ODS may request additional information depending on the accommodations you are requesting.
